Level of fidelity (percentiles):

• Low fidelity: 0–50 (variance > = 50%)

• Moderate fidelity: 51–79 (variance between 21–49%)

• High fidelity: 80–100 (variance of 0–20%).

Fidelity Audit Observation Tool, used once between 6–9 months into implementation.

Fidelity Self-Rating Tool, used between 6–9 months into and after implementation completion.
